A pathology case review can be requested by shipping the specimens (along with their appropriate documentation as detailed below) directly to our laboratory.

Requisitions

Required Items for Pathology Consultations:

  • A completed requisition w/patient demographics
  • Patient insurance and/or client billing information
  • Cover letter to include pathology report, slides/blocks, requesting physician contact and facility information

Slides and Blocks
We use FedEx Ground for routine returns of slides and blocks. Expedited return services are available when a FedEx or UPS account number is provided by the requester.
